Health Insurance TAX Documents for 2016
Soon you will receive either form number 1095-A or 1095-B from the NYS Health Insurance Marketplace/Exchange.
These forms should be given to your tax preparer for your 2016 tax return.
1095-A will be mailed from the New York State of health for all individuals enrolled in the Bronze, Silver, Gold, or Platinum Plan on the exchange. Forms can also be downloaded from your main account holders NY State of health inbox.
Individuals enrolled in the Catastrophic Plan, The Essential plan, Medicaid, or Child Health Plus will NOT received a 1095-A because these plans are NOT eligible for tax credits. Rather you will be mailed a 1095-B form. 1095-B form are NOT available on the NY state health inbox.
